By addressing advance care planning during clinic visits and providing patients with guidance and support, Gundersen Health System based in La Crosse, Wisconsin, gives patients care that is aligned with their wishes and helps reduce costs. As a result of this program, La Crosse has become known as “the best place to die in America.” A study from 2007 showed that roughly 96% of La Crosse residents who died in the previous year had a completed advance directive.
