Flip the Script on Vaccinations and Reach the Kids Who Don’t Show Up at Your Door

At a time when pediatric vaccination rates were falling in the U.S., Nemours increased those rates by analyzing care gaps and designing creative care models.
Nemours Children’s Health, a pediatric organization with sites in Delaware, Pennsylvania, New Jersey, and Florida, made closing vaccination gaps a priority as it transitions from a fee-for-service model to a value-based care model. A multidisciplinary team of clinicians, administrators, and analysts came together in Nemours’ Value-Based Services Organization (VBSO) to analyze where the communities it served had gaps in vaccine coverage and to develop strategies to fill those gaps. As a result of these efforts, Nemours saw relative increases of 13% in children vaccinated for flu and 148% in children who received their first HPV vaccination before their 11th birthday in 2020 compared to 2019.
How They Did It:
Used a patient registry to define a broader population of patients
Queried state vaccination registries for up-to-date information on vaccinations
Targeted outreach to patients who needed preventive care
Combined telehealth well visits with in-person “miniwell” visits for vaccination
