Memorial Healthcare and Orange Care Group Partner in Population Health

For independent physician organizations that are part of an accountable care organization (ACO) or other value-based care agreement, measuring patient outcomes and staying in the loop when patients receive care elsewhere is critical. That’s why Orange Care Group, a collaborative group of ACOs and value-based care organizations, is partnering with Florida’s Memorial Healthcare System to provide Healthy Planet tools to its 600+ independent providers across seven states in the first partnership of its kind.

Connecting with Memorial Healthcare System, itself a clinically integrated network focused on value-based care, gives Orange Care Group’s independent providers access to a single patient record shared with Memorial.
